About Us
HH Collaborative PCN was formed by 8 GP practices with the aim to improve the health care in Hayes and Harlington area.
See our practices
GP practices work together in order to standardise the care across the practices in our area. We work in close collaboration with other health care providers such as the community pharmacists and the community mental health team. We are also working in partnership with Hillingdon Council for health promotional work such as weight reduction and smoking cessation. Our primary care network brings together the experience and skills of its doctors, nurses, pharmacists, practice managers and other non-clinical staff. We work closely with our Patient Participation Group to deliver appropriate services.
Our vision is Improving the life expectancy of south of borough comparable to the north of the borough.
We want to improve health inequalities within our registered population.
We want to provide optimum preventive health care such as vaccination, cancer screening, and early identification of high blood pressure and Diabetes.
We are working on improving access to health care by promoting e-consultation and Text messaging.
We invested in Information and Technology and use it to identify areas of improvement.
We have a financial and operation structure in place.
We have an active Patient Participation Group and have regular meetings.
We employ alternative workforce in order to provide effective health care. Examples are Pharmacists, First Contact Physiotherapist, Health and wellbeing Coach, Social and Link Workers, Paramedics, Dietitian and Care Coordinators.
We also invest in education and training. Our workforce is engaged in regular training and improvement.
Each practice still maintains their autonomy in providing care for their registered patients and they have access to all health care provided by our PCN.
